Clinical Triage & Prioritization Policy

Inclement Weather & Travel Safety Policy

At Trusty Triage, the safety of our community nursing team and our clients is our top priority. Due to the unpredictable nature of Ontario winters and severe weather events, the following policy applies to all In-Home Wound Care visits (effective September 1st, 2026. Postponement of In-Person Visits: In-home nursing visits may be postponed or rescheduled if weather conditions make travel unsafe. This includes, but is not limited to:

  • Severe Winter Weather: Blizzards, heavy snowfall, or significant ice accumulation
  • Environment Canada Alerts: Official Red or Orange weather warnings for the Chatham-Kent or Moraviantown regions.
  • Road Closures: Any police-mandated road closures or “essential travel only” advisories

Virtual Continuity: The virtual Bridge

If an in-home visit is postponed due to weather, we will not leave your care unattended.

  • Automatic Transition: your scheduled in-person visit will be converted to a Virtual Assessment via Jane Telehealth
  • Remote Guidance: Our RNs will use Swift Medical AI to guide you or your caregiver through a self-dressing change using your Cardinal Health supplies
  • Vitals Review: We will perform a full review of your CloudDx biometric data to ensure clinical stability until a safe in-person visit can be completed.

Notification Policy

  • Communication: If weather impacts your visit, we will notify you via SMS or email through Jane App at least two (2) hours prior to your scheduled time.
  • No Cancellation Fee: If TrustyTriage postpones a visit due to weather, the $50.00 Missed Appointment Fee will not apply.

Client Responsibility

For the safety of our nurses, please ensure your driveway and walkways are cleared of snow and ice prior to a scheduled in-home visit. If a nurse deems a property’s access point unsafe, the visit may be converted to a virtual assessment.

After-Hours and & Clinical Alert Policy

While our clinical team operates during scheduled business hours, your CloudDx RPM kit remains active 24/7:

  • Automated Flags: If your vitals (blood pressure, SpO2, heart rate or temperature) exceed the personalized thresholds set by your RN, the system will automatically flag these readings in our clinical dashboard.
  • Morning Review: Our Virtual Clinic Lead RN reviews all after-hours flags at the start of the next business day (Monday-Friday, 9AM-9PM)
  • Automated Client “Ping” (Safety Alert): If the CloudDx system detects a reading that is significantly outside of your safe range, you will receive an automated notification (Push Notification or Email) directly from the device.

Cancellation Policy:

Your appointment time is reserved just for you. While we understand that things come up, a late cancellation or missed visit causes a hole that could be filled by another client. As such, we require 24 hour notice for any cancellations or changes to your appointment. Clients who provide less than 24 hour notice, or miss their appointment, will be charged a cancellation fee of $50 to the card on file.
