Step 1: Choose your appointment

Click the Book Now button to be directed straight to our secure Jane App portal.

• Select Initial Virtual Assessment: This initial consultation is your entry point. Our nursing team will use their clinical expertise to evaluate your needs and determine the best care plan.
• Select your Preferred Window: Pick a day and time black that works best for your schedule.

Step 2: Complete your Digital Intake

• Once you select a time, you will receive a secure email link to complete your Master Intake Form. To ensure a seamless onboarding experience, please have the following ready:
• Insurance Details: A photo of your private or extended insurance card (front/back). We use this to verify your coverage for your in-home Cloud DX Connected Health™ kit and advanced wound supplies.
• Credit Card: You can choose to have a card on file to automate your ongoing treatment plan billing cycles.
• The Comfort & Safety Agreement: You will digitally sign our standard clinic service policies, including our mandatory 1-hr no smoking rule and pet safety policy, to protect our field nurses.

Step 3: Join your virtual clinic

On the day of your initial appointment, you will receive a unique, secure link sent directly to your phone via email or SMS.

• One-Click Entry: There is no complicated software to download. Simply click the link from any smartphone, tablet, or computer to join the private Jane Telehealth room.
• Clinical Tech & Onboarding: Your nurse will guide you through your intake form if required. Following the session, our logistics partners handle shipping your hardware directly to your door and providing full support setup.

Important Billing Information

• Initial Assessment: This first visit is billed directly to insurance when eligible. If you wish to pay the fee up front and submit to insurance later, we are set up with auto deposit so no security question is needed. An automated receipt will be instantly pushed to your Jane Client Portal, allowing you to submit it to your insurance for reimbursement.
• Ongoing Care: Once your initial assessment is complete and you transition to one of our ongoing monthly care tier plans (such as the specialist bridge, chronic wound, or our hospital-at-home plan), all recurring fees will be billed to insurance (Greenshield ) where eligible.
