We offer continuous virtual ward admission: 24/7 digital safety net providing active nursing oversight using Swift Medical AI. Daily biometric monitoring via our RPM kits – vital tracking (BP, SpO2, Temp) to identify system changes.
Our nurses will visit you virtually a total of 2 times a month to review biometric data, medication tracking and monitor for infection.

Perfect for clients that have no doctor or nurse practitioner following their health journey. We provide 24/7 biometric health monitoring and medication tracking in one comprehensive chart. Just let us know when you find someone and we send your chart to them!
Our RPNs will visit you a total of 2 times a month. The first visit will be in-person and the 2nd visit will be virtual to review biometric data, check manual vitals, and ensure no “red flags” have appeared since the physical visit.
SPECIALIST BRIDGE TREATMENT PLAN – Contact us for Price (billed directly to insurance)
Bridging the gap for clients awaiting hospital or specialist intervention. It includes intensive pre-surgical wound bed optimization. Infection monitoring provided through our RPM Kits to prevent ER visits while on surgical waitlists. Clinical advocacy and coordination of care with CKHA so you don’t have to.
Our RPNs will visit you a total of 4 times a month. 2 in-person visits, and 2 virtual visits, alternated.
CHRONIC WOUND TREATMENT PLAN – Contact us for Price (billed directly to insurance)
Perfect for those long-term maintenance or non-healing wounds. We provide weekly 4K virtual triage. Asynchronous wound tracking is provided to monitor for stalling or deteriorating wounds. Daily clinical progress reports are provided after each appointment to your primary Circle of Care.
Our RPNs will visit you a total of 16 times during the month 12 in-person visits, 3x a week, and 4 virtual check-ins once a week.
HOSPITAL-AT-HOME TREATMENT PLAN – Contact us for Price (billed directly to insurance)
60-day specialized transition of care for high-risk surgical recovery per Physician Orders.
We provide a safe, clinical bridge for clients transitioning from the hospital to home. We combine advanced remote monitoring technology with frequent, hands-on clinical oversight to prevent readmission and ensure a stable recovery.
Our RPNs will visit your home a total of 8 times (2 per week) during the month to perform physical exams, manage complex wound care, and ensure your environment is optimized for healing. Simultaneously, our RNs will provide weekly virtual assessments to monitor your biometric data. In the second month, you will transition to a weekly virtual check-in with our registered staff to ensure long-term stability.
