TIER 1 TREATMENT PLANS
VIRTUAL NURSING TREATMENT PLAN – billed directly to insurance – contact us for a benefit check
We offer continuous virtual ward admission: digital safety net providing active nursing oversight using AI.
What’s Included:
Virtual Membership: $37.25/week. (Total is $149/month).
Clinical Support: Includes 9am-9pm messaging support, review of your health data to identify system changes early. (Please Note: Hardware RPM kits are exclusive to our Premium Tier 2 & 3 Plans.
Our nurses will visit you virtually a total of 2 times a month to review your progress, biometric data, medication tracking and monitor for infection.
ORPHAN CLIENT TREATMENT PLAN – Eligible for direct billing via ProviderConnect
Perfect for clients who have no doctor or nurse practitioner following their health journey. We provide 24/7 biometric health monitoring and medication tracking in one comprehensive chart. Just let us know when you find someone and we send your chart to them.
Orphan Client Membership: $47.25/week (Total: $189/month).
Clinical Support: Includes primary care advocacy, 9am-9pm messaging support, and all nurse visits within your plan.
Our RPNs will visit you a total of 2 times a month. The first visit will be in-person and the 2nd visit will be virtual to review biometric data, check manual vitals, and ensure no “red flags” have appeared since the physical visit.
TIER 2 TREATMENT PLANS
SPECIALIST BRIDGE TREATMENT PLAN – Eligible for direct billing via ProviderConnect
Bridging the gap for clients awaiting hospital or specialist intervention. It includes intensive pre-surgical wound bed optimization and proactive infection monitoring provided through our RPM Kits to prevent ER visits while on surgical waitlists. We provide clinical advocacy and coordination of care with CKHA or your local hospital so you don’t have to.
Specialist Bridge Membership: $150/week Clinical Support: Includes advocacy and coordination with specialists, 9am-9pm messaging support, biometric oversight, and all nurse visits in your plan).
Our RPNs will visit you a total of 4 times a month. 2 in-person visits, and 2 virtual visits, alternated.
TIER 3 TREATMENT PLANS
CHRONIC WOUND TREATMENT PLAN – Eligible for direct billing via ProviderConnect
Our most intensive program for non-healing or stalling wounds. We provide an unprecedented 16 nursing visits per month to ensure complete oversight. By combining 4K virtual triage and daily AI-powered tracking, we identify red flags before they lead to an ER visit. We don’t just manage your wound; we drive it toward closure.
Chronic Support Membership: $650/week (Includes intensive 4K virtual triage, daily AI-powered red-flag tracking, 24/7 biometric safety-net monitoring, 9am-9pm messaging support, and all 16 clinical visits in your monthly plan.)
Our RPNs will visit you a total of 16 times during the month 12 in-person visits, 3x a week, and 4 virtual check-ins once a week.
HOSPITAL-AT-HOME SPECIAL
HOSPITAL-AT-HOME TREATMENT PLAN – Eligible for direct billing via ProviderConnect
Our gold-standard recovery program. We provide a 60-day clinical bridge for high-risk surgical patients. By combining 16 in-home physical exams with daily biometric dashboard oversight and structured virtual RN check-ins, we ensure recovery is on track and prevent the complications that lead to hospital readmission. Includes full biometric monitoring for the duration of the program.
Hospital-at-Home Acute Membership: $400/week (Includes intensive short-term clinical oversight, 24/7 biometric safety-net monitoring, priority 9am-9pm messaging, and all integrated clinical visits in your 8-week acute recovery plan.)
Our RPNs will visit your home a total of 16 times (4 per week) during the first month to perform comprehensive physical assessments, manage complex wound care, and ensure your environment is optimized for healing. This care is tightly reinforced by continuous daily biometric monitoring and 4 dedicated virtual check-ins during the second month(scheduled weekly) to maintain absolute safety and medical continuity at home.
